Student Coordinator (full-time)

Under the direction of the Medical Director, the Student Coordinator will work with the Student Director to develop and operate programs to engage students in primary care for the underserved, and to recruit providers for Christ Community Health.

Nurse Manager (RN)

The Nurse Manager is responsible for performing patient care within the scope of nursing practice under the direction of the Medical Director. The Nurse Manager is responsible for organizing policies and procedures for personnel in direct contact with patient care.  The Nurse Manager supervises and manages the duties of Staff Nurses (RNs/LPNs) within the scope of nursing, as well as the Clinical Facilitators and their duties. Must fully embrace our Mission and Vision.

Medical Scribe (Intern)

This internship is for students taking a gap year before going to a health profession school (medical, public health, and physician assistant) and who have a vision for primary care among the poor. The intern reports to the Physician leading their care team and will assist with a variety of scribing, documentation, clinical and administrative tasks or projects. Job assignments will vary by the current needs of the clinic.

Quality Improvement Assistant (Intern)

This internship is for students taking a gap year before going to a health profession school (medical, public health, and physician assistant) and who have a vision for primary care among the poor. Patient Services Manager (full-time)

Christ Community Health Services is seeking a highly motivated and passionate Patient Services Manager with at least 5 years of patient registration experience to fill a newly redefined role. The Patient Services Manager is responsible for overseeing all phone support, scheduling, and check-in/check-out functions.

With the opportunity to make an immediate impact, this person should be a self-starter who can take a step back and see the big picture, while also “thinking on your feet” and making in-the-moment decisions. The ideal candidate will demonstrate the ability to cast strategic vision, identify and execute priorities, and build consensus among a variety of stakeholders. Behavioral Case Coordinator (full-time)

The Behavioral Case Coordinator/Paraprofessional will provide the full scope of primary care services which fall under his/her field of training, including but not limited to following up with patients via phone, engaging with patients about their treatment goals, providing some psychoeducation when appropriate, and assisting clients with following up with and assessing the effectiveness of referrals. Care Team Nurse (RN-LPN) (part-time, 3 days per week)

The Care Team Nurse (LPN/RN) works within a Care Team to provide Nurse Visits, Triage, Case Management, patient education, and patient care support to the Care Team within the scope of nursing. Must fully embrace our Mission and Vision.

Primary care providers in Internal Medicine, Family Medicine, and/or Pediatrics (full-time)

Must be able to manage complex medical issues in highly uninsured, low-resource, populations. Ability and willingness to provide inpatient care is desired. Long-term commitment to community, teaching, and discipling valued. Willingness to live in neighborhood served is valued. Must fully embrace our Mission and Vision. Opportunity for HIV disease management and program development for prenatal care primarily for Spanish speakers.
