Behavioral Health Services are available for all patients – adult and pediatric. As members of your care team, behavioral health providers are available to help you address mental health and spiritual concerns, cope with chronic illness and life problems, and assist with making positive life changes. Your PCP and your behavioral health provider work together to improve your physical, mental, behavioral, and spiritual health and well-being. Patients exceeding the scope of care at CCH will be referred to a specialty mental clinic.

Prescription Information
Prescription refill requests must come from your pharmacy.
To get a prescription refill, first call your pharmacy 3-5 days before you will run out of your medicine. If you have no refills, ask the pharmacist to send a refill request to our office.
If your provider needs to see you, someone from the clinic will reach out to schedule an appointment.
For refills on controlled substance prescriptions, please log into the patient portal or call (706) 922-0600.
